The Department of Nursing is under the direction of a qualified nursing administrative staff. The ultimate responsibility for the functions of the Nursing Department rests with the Director of Nursing (DON). The nursing staff and the interdisciplinary care team in conjunction will develop a plan of care with your attending physician. The Nursing Department makes certain that qualified nursing personnel are on duty 24 hours a day, seven days a week. In addition to the administrative staff and professional nurses, each nursing unit is staffed with appropriate numbers of Certified Nursing Assistants. Staffing patterns and unit assignments are based upon the level of care required by the patient, and consistently exceed regulatory guidelines. Your room is equipped with a call bell system. You are encouraged to use your call bell to request assistance when needed. Increasing and maintaining your independence is a vital component of your care. Our focus is, and will always be, on promoting your optimal level of functioning.
We offer skilled short-term rehab to assist those who, after a hospital stay or surgery, are in need of a sound rehabilitation program before returning home. Our comprehensive rehabilitation program includes physical therapy, occupational therapy, speech therapy, post-surgical care, IV therapy and a host of other rehab services that are designed to support the transition from hospitalization to home.

The Social Services department is comprised of full-time, professional, and educated Social Workers who are available to provide direct clinical and supportive services to both you and your family. The primary role of the Social Worker is to aid you in realizing your highest level of social and emotional functioning in your new environment, considering your current physical and cognitive status. Social workers are accessible to help meet family needs through a supportive relationship, but also through counseling services. In addition to assisting you and your family and addressing the various concerns or problems that may arise, the Social Service Department staff also: Assist residents and families in adjusting to the home Respond to discharge planning needs of residents Direct residents & families to community information and resources as necessary Assist families as needed in making the financial transition to medical assistance Serve as a liaison between residence/families and other staff to advocate for residents’ rights Provide crisis intervention when immediate concerns surface
A comprehensive interdisciplinary care plan is developed to meet your specific needs shortly following admission and periodically thereafter. You, your family, and/or your responsible party are encouraged to attend the care conference meetings at which time the care plan, as well as other issues such as expectations, are discussed. These meetings provide a formal opportunity to ask questions of those staff directly involved in your care. Communication will be sent to your family and/or responsible party to notify them of the date and time of the conference. You will be invited to attend the care conference. In the event your family members are unable to attend scheduled meeting times, arrangements can be made for telephone conferencing.
Hospice and Palliative services are available to support residents and families during this most sensitive time. One of the primary goals of our end-of-life care is to assist the individual to remain as pain-free as possible while maintaining personal dignity. Pastoral care support is also a fundamental part of this program to address spiritual needs.

At Resorts at Pooler, we believe that each resident is an individual with unique interests and personality. The Activities Director of Resorts at Pooler has created programs that are geared toward each resident’s emotional, intellectual, physical, social and spiritual needs. Working together with our other departments, the activity department gives our residents the opportunity to participate in programs that are meaningful and relevant to their interests while allowing them to reinforce the skills and tools they have developed in their recovery process. Our Activities Director designs a flexible calendar, with a variety of stimulating and engaging activity options. We have a program to fit most anyone's needs; group or individual programs, craft programs, sensory programs, intellectual programs, physical programs as well as religious programs.
The dining experience is an integral part of daily life. The Dining Service Department provides a wide variety of nutritious meals, snacks and nourishment's according to the diet approved by your physician. Upon admission, a dietitian will evaluate your nutritional needs while also considering your personal preferences. Menus are posted daily for residents to view. Alternatives are available for each meal and residents are encouraged to request these alternative selections as they wish. Throughout the year the Dining Service Department offers special themed meals, holiday menus, cocktail hours, picnics and monthly parties for residents celebrating their birthdays. Residents’ opinion and suggestions are incorporated whenever possible into our dining program. Regular inspections are conducted to ensure your dietary needs are being satisfied and that food is being served at proper temperatures. You are welcome to invite a guest to dine with you. We offer meals to guests upon request.
